Printable Blood Pressure Log
A simple paper log for tracking your readings by hand — print it, fill it in, and bring it to your next appointment.
| Date | Time | Systolic | Diastolic | Pulse | Notes |
|---|---|---|---|---|---|
Tips for an accurate log
Measure at the same times each day, seated and relaxed, with your arm supported at heart level. Wait at least 30 minutes after caffeine, exercise, or smoking. Take two readings a minute apart and note both, or just the one that feels representative.
Typing every reading by hand gets old fast. SnapSana reads the numbers straight off your monitor's display with your camera — no paper, no typing, and your trends are calculated automatically. See how it works →
What to do with a completed log
Bring your log (or a SnapSana PDF export) to your next appointment so your doctor can see the full pattern, not just one reading. See our blood pressure chart for what the categories in each reading mean.
This log is for your own records and is not medical advice. Talk to your doctor about your blood pressure.
